TRANSITIONS SERVICE

We are committed to providing young people to have specialist multi-disciplinary care if they require support up to the age of 20 years old. Often young people who are between sixteen to twenty struggle to find services which are developed with this age group in mind. Our team are experienced in working with local professionals, including GP's and other services to help ensure that all young people receive the care they need. This includes liaison with colleges and universities and also providing care which is not provided locally where the young person is living.

We see young people presenting with:

ASSESSMENTS WE PROVIDE

  • Adjustment Reactions

  • Anxiety Disorders

  • Attention Deficit Hyperactivity Disorder (ADHD)

  • Body image disturbance

  • Depression

  • Obsessive Compulsive Disorder

  • Psychosis

  • Self-esteem issues

  • Self-harm

  • Trauma and Post-Traumatic Stress Disorder.

  • We have the skills and expertise to link in with local services if we assess that more support is required.

We offer a multi-disciplinary service including Psychiatry, Psychology, Occupational Therapy and Educational Specialists. This means that young people can access support which is personalised and help them develop their resilience, independence and coping strategies.
